Managing Demodex blepharitis and meibomian gland dysfunction with light therapy
Demodex blepharitis and meibomian gland dysfunction, often shortened to MGD, can cause sore, irritated, gritty and uncomfortable eyes. In some patients, these conditions can also lead to repeated flare-ups of inflammation around the eyelids and the surface of the eye.
This patient case, shared by our own Sarah Farrant, shows the impact of a combined light therapy treatment plan for a 73-year-old patient with Demodex blepharitis, MGD, recurrent marginal keratitis and a high level of lid margin debris. The patient received four treatment sessions using red and blue low-level light therapy alongside IPL treatment.
What was happening before treatment?
Before treatment, the patient’s meibomian glands were producing thick, cloudy oil. These glands sit along the eyelids and help keep the tears stable. When the oil becomes too thick, it can block the glands and make dry eye symptoms worse.
There were also clear signs of Demodex blepharitis. Demodex mites are tiny organisms that live around the eyelashes. In small numbers they are common, but when there are too many, they can contribute to inflammation, irritation and a build-up of waxy debris around the base of the lashes.
The patient also had foamy material along the eyelid margin. This can be a sign of increased bacterial activity and poor oil quality, and in this case it was linked with repeated episodes of marginal keratitis.
What changed after four sessions?
After four combined treatment sessions, the oil from the meibomian glands appeared clearer, thinner and easier to express. This suggests the glands were functioning better and producing a healthier oil layer for the tears.
The waxy debris around the eyelashes was also noticeably reduced. This is important because lash debris is one of the key signs of Demodex blepharitis. A reduction in this build-up can help calm the eyelids and reduce irritation.
The foamy material at the lid margin had resolved completely in this case, suggesting an improvement in both bacterial load and meibomian gland function.
How does the treatment help?
The treatment combines different forms of light therapy, each with a different role.
IPL treatment uses controlled pulses of light to warm and loosen thickened oil in the meibomian glands. It can also help reduce inflammation around the eyelids.
Red low-level light therapy supports cell repair and may help the glands return to healthier function.
Blue low-level light therapy has an antimicrobial effect, helping to reduce the bacterial load around the eyelids.
Together, these treatments aim to improve the quality of the oil in the tears, reduce eyelid inflammation, lower Demodex-related debris and support a healthier, more stable tear film.
Why this matters for patients
For patients with ongoing dry, sore, gritty or inflamed eyes, the problem is not always just “dry eye”. Eyelid inflammation, blocked meibomian glands, Demodex mites and bacterial imbalance can all play a part.
This case shows how a targeted treatment approach can help address several contributing factors at the same time. While every patient is different and results vary, a detailed assessment can help identify what is really driving the symptoms and which treatment options may be most appropriate.
